I certify that the above information is true and correct. I agree to the release of any information concerning my records at the University of Houston to any federal, state, or private agency necessary for the administration of the scholarship program. I authorize the scholarship committee to access my academic records for the purpose of evaluating my qualifications for a scholarship through the Conrad N. Hilton College of Hotel and Restaurant Management. I also authorize the scholarship committee to verify any and all information contained in this application, including dates of employment, employment status, and honors.

I understand that if I am selected to receive a scholarship, there are obligations I will be required to meet, including attending a scholarship orientation, a scholarship dinner, and other events which may be requested by my donor or the college (i.e. meetings, luncheons, dinners, etc.) By not meeting these obligations I understand that my scholarship may be affected. I am aware of the policies regarding scholarship administration. They can affect the amount of my scholarship award and my degree planning.
I must apply for scholarships each year. There are no automatic renewals.
I understand that I will receive ½ of the award in the Fall Semester and ½ of the award in the Spring Semester.
If I do not accept the award during the Fall Semester, only ½ of the original award can be received if I return to school in the Spring Semester.
If I receive an award for the academic year and I attend only the Fall Semester, the award will be halved.
If I am unable to accept an award for the academic year in which the award was made, I will have to reapply to be reconsidered.
I understand that if my contact information changes, I must notify the scholarship committee as soon as possible.
To receive full scholarship benefit I must maintain 12 or more credit hours as an undergraduate and 9 or more hours as a graduate. If I am graduating at semester end and fall below this minimum, my semester scholarship award will be prorated based on credit hours remaining in my degree plan (semester award divided by required credit hours multiplied by actual credit hours).
My Failure to meet a deadline or obligation may result in a $100.00 deduction in award for each failed deadline or obligation. Some examples of Deadlines or Obligations are:
- Late Application
- Failure to return agreements by deadline
- Failure to attend orientation or other required meetings
- Failure to submit thank you letters on time
- Failure to attend designated events
- Failure to insure updated contact information
